I seem to always have bad breath, that sulfury type, no matter what I do.  I floss daily (and when I floss I get a whiff of the smell), brush my tongue so hard I sometimes gag, and of course brush my teeth twice a day at least.

What is weird is that my girlfriend does not brush her tongue every day (she does sometimes) and only flosses occasionally, and yet, her breath is usually perfectly fine when I kiss her.  Is there some disease that makes some people just have bad breath no matter what?

You need to talk to a doctor, because sometimes GI issues can cause this. Talk to your dentist and your doctor to rule out medical/physical issues.
